Our policy to use of Zakat is based on Fatwa (Edict) from a Mufti and have followed this Fatwa. The original copy, which is in Urdu, can be downloaded. 

Following the Fatwa we have established a separate Zakat Account and all donations for Zakat are banked in this account. beneficiary who arrive at our clinic and claim that he/she is Zakat eligible then beneficiary is given this form to read or we explain if they cannot. that they want Al-Mustafa Trust to administer their healthcare from this fund on their behalf. If they agree they have to sign this document.
